ABSTRACT Self-medication among adolescents constitutes a significant public health concern, particularly due to the risks associated with the inappropriate use of medicines and the influence of unreliable sources of information. This study aimed to assess the level of knowledge, perceptions, and sources of information regarding the rational use of medicines among high school students from two public schools in São Luís, Maranhão, Brazil, in order to identify knowledge gaps and support the development of health education strategies. This was a quantitative, descriptive, cross-sectional study. Data were collected through a structured questionnaire administered to students, addressing sociodemographic characteristics, knowledge about medicines, rational medicine use, sources of information, and risk perception. Data analysis was performed using descriptive statistics and Pearson's and Spearman's correlation matrices. The results showed that the students demonstrated satisfactory knowledge regarding the importance of reading package inserts, completing antibiotic treatment, and the risks associated with self-medication. However, significant knowledge gaps were identified concerning drug interactions, the proper disposal of expired medicines, and the influence of pharmaceutical advertising. The main sources of information reported were family members, healthcare professionals, and the Internet. It is concluded that, despite the partial knowledge demonstrated by the students, important weaknesses remain, highlighting the need for continuous health education initiatives in the school environment to promote the rational use of medicines and reduce inappropriate self-medication.pt_BR
