Inibidores da SGLT2: Uma revisão dos impactos na modificação da mortalidade por insuficiência cardíaca de fração de ejeção reduzida

Abstract

RESUMO: A insuficiência cardíaca com fração de ejeção reduzida (ICFER) é uma patologia frequente no cenário brasileiro e responsável por significativa morbidade e mortalidade. Nesse contexto, o presente trabalho objetiva a revisão integrativa da literatura acerca dos inibidores da SGLT2, um dos medicamentos mais recentes incluídas em diretrizes de tratamento dessa doença. Para tal, foi realizada uma pesquisa bibliográfica nas bases de dados PubMed e BVS, no intervalo temporal de 2015 a 2024, utilizando os descritores: “SGLT2 inhibitor” e “reduced ejection” e “heart failure”. Dessa forma, artigos foram selecionados e lidos integralmente para compor a discussão. Como resultados, selecionou-se 16 artigos, os quais abordam a potencialidade do uso de inibidores da SGLT2 em pacientes com ICFER, ainda que não possuam diabetes mellitus tipo 2. Outro ponto importante é que esses medicamentos podem reduzir hospitalizações e morte cardiovascular. Os dois estudos recentes mais importantes para o contexto brasileiro, onde são comercializadas a dapaglifozina e empaglifozina, foram o DAPA-HF e EMPEROR-Reduced. Os resultados desta pesquisa poderão auxiliar os médicos, especialmente cardiologistas, na obtenção de informações atualizadas sobre a temática, podendo embasar suas escolhas no uso dos iSGLT2 como ferramenta no manejo da insuficiência cardíaca com fração de ejeção reduzida, por conseguinte impactando positivamente no tratamento dos pacientes. Ademais, também poderá servir de base para a criação de políticas públicas de saúde no Brasil.___ABSTRACT: Heart failure with reduced ejection fraction (HFrEF) is a common pathology in Brazil and is responsible for significant morbidity and mortality. In this context, the present work aims to review the literature on SGLT2 inhibitors, one of the most recent medications included in the treatment guidelines for this disease. To this end, a bibliographical search was carried out in the PubMed and BVS databases, in the period from 2015 to 2024, using the descriptors: “SGLT2 inhibitor” and “reduced ejection” and “heart failure”. Therefore, articles were selected and read in full to form part of the discussion. As a result, 16 articles were selected, which address the potential use of SGLT2 inhibitors in patients with HFrEF, even if they do not have type 2 diabetes mellitus. Another important point is that these medications can reduce hospitalizations and cardiovascular death. The two most important recent studies for brazilian context, where dapaglifozin and empaglifozin are marketed, were DAPA-HF and EMPEROR-Reduced. The results of this research will be able to help doctors, especially cardiologists, in obtaining updated information on the subject, being able to support their choices in the use of iSGLT2 as a tool in the management of heart failure with reduced ejection fraction, therefore positively impacting the treatment of patients. Furthermore, it could also serve as a basis for the creation of public health policies in Brazil.
